NND. Teeth overall clean – retained decid K9 upper right with calculi build up between. Fractured left lower K9 at base. Integ: Several matts throughtout – worse at paws (not encircling) and hind end. Urine and fecal staining at hind end. Perineal region moderately erythematous. Overgrown nails x4 H/L auscult WNL Abd snp/nmp. No MGTs A: 1 yo FI Shih Tzu Poor haircoat with matting and urine/fecal stain, overgrown nails Otitis externa AU Dental disease Mild ocular irritation P: Signed up for grooming with matts clipped – at risk for developing urine scald, early signs present but should resolve if grooming addressed promptly Signed up for ear cleaning and nail trim Rx otibiotic AU BID x7 days Reco placement ASAP for proper husbandry, follow up care if needed. Reco full ocular work up with placement. Prognosis good Activyl 1 ml topical Pyrantel 1 ml PO Gave RV, DA2PP, CIV, Bordetella
