Medical Summary DVM Intake Exam Intact female Estimated age: 10 years Microchip noted on Intake? negative History : Patient is stable. Stray dog. Subjective: Geriatric underlying intact female dog with matted hair. Observed Behavior – Scare. Submissive. Allowed handling. Objective P =130 bpm BCS 5/9 EENT: Eyes – matted hair around the eyes, ears clean, no nasal discharge noted Oral Exam: dental disease PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: intact male MSI: Ambulatory x 4, skin free of parasites, no masses noted, matted , fllacky hair coat CNS: mentation appropriate – no signs of neurologic abnormalities. Assessment Matted hair Plan CBC/Chem/TT4 should be performed Total body shave ( sedated) 50 mg cefpodoxime sid po for 7 days. Prognosis: good Permanent waiver due to age
