Medical Summary DVM Intake Exam Estimated age: 3yr Microchip noted on Intake? no History : O/S Subjective: Dog is BARH Observed Behavior – Highly fearful, tries to escape, avoids interaction even with a slow approach and time to warm up. Restraint (with large blanket, unable to muzzle) needed for exam and vaccines. Dog escalated to lunging and snapping whenever approached. Mingo is a 3 year old unaltered male Small dog, he came to his owner from a previous owner as a puppy and is being surrender due to behavior issues. He has no known health issues or injures and has no recent vet history.
Around strangers Mingo is barks and becomes tense when the approacgh him. He has lived wit a 6 year old child and has snapped at the child whenever she tries to pick him up. Around other dog s he growls and barks if he sees them. Mingo has bitten a person Before but did not draw blood.
Mingo is Partially housetrianed but still has accidents in the home a few times a week. Mingo will Growl or snap if some tries to touch her food bowl. He is very tense during a bath and When a unfamiliar person approches the home he will bark, growl and bite. He was described as shy, Pushy and excitable with a high activity level.
He likes to play ball and squeaky toys, he likes to sleep in the kitchen with a blanket . Mingo eats dry food with pices of chicken or beef, When left alone he is well-behaved abd has never been crate trained. Mingo goes on brisk walks on leash and will run away if his is taking off leash.
Upon intake Mingo was timid and growled at the counselor when they approaced him he was scanned and collared by his owner.

Bite history: Yes, Mingo bit on two separate occasions. Both victims were friends of the family visiting the home at the time. Mingo became tense, bared his teeth, and lunged biting both victims on the ankle.

SAFER ASSESSMENT: 8/16/17
Summary: Mingo is difficult to remove from the kennel, lunging and snapping at the leash. He appears to be uncomfortable or afraid of the slip lead. He has also allowed minimal handling from his caretakers. Out of concern for his stress levels and his response to restraint, we feel Mingo is not a great candidate for a handling assessment at this time. PLAYGROUP:
According to Mingo’s previous owner, Mingo growls and barks if he sees other dogs in the streets. When off leash at the Care Center, Mingo greets a group of small male and female dogs with a high vibrating tail and soft body. He is slightly sexually motivated, but does not mount.

INTAKE BEHAVIOR
8/14/17: Upon intake Mingo was timid and growled at the counselor, no handling was conducted due to his behavior.
MEDICAL BEHAVIOR:
8/14/17: During his medical examination, Mingo was highly fearful, tries to escape and avoids interaction. He lunges and snapped when being approached.
RECOMMENDATIONS: New Hope Only

_X_Place with a New Hope partner- Mingo has not acclimated well to the kennel environment and has allowed only minimal handling since intake. We recommend placement with a New Hope partner who can provide any necessary behavior modification (force-free, positive reinforcement-based) and re-evaluate behavior in a stable home environment before placement into a permanent home.
Potential challenges:
_X_Fearful/potential for defensive aggression- Mingo has attempted to bite during his stay in our care centers when uncomfortable; guidance from a professional trainer/behaviorist is recommended to assess behavior after decompression in a new home environment. Force-free, reward based training is advised when introducing or exposing Mingo to new and unfamiliar situations.
