Medical Summary 05/17/17 DVM Intake Exam Estimated age: 8 yrs Microchip noted on Intake? No. History : Stray dog, no medical concerns noted. Subjective: Observed Behavior – Friendly and social, very interested in her surroundings. Allows all handling. . Objective: BARH. mm=pink, moist, CRT<2 s. HR=112, RR=20. BCS=6/9. ORAL: Mod. calculus, gingivitis. EENT: Eyes clear, ears clean, no nasal discharge noted. PLN: No enlargements noted. H/L: Lungs clear. Gr 3/6 systolic left sided cardiac murmur. No dyspnea, no coughing. ABD: Non painful, no masses/organomegaly palpated. U/G: Female, spay scar found. MS: Ambulatory x 4, no lameness. INTEG: Full, clean, well groomed haircoat. No skin lesions. CNS: Mentation appropriate – no signs of neurologic abnormalities. A: Adult dog with dental disease and heart murmur. Heart murmur ddx: congenital, acquired valvular disease. P: Monitor while at BACC. Good prognosis. Rec. cardiac consult with ultrasound when placed.
