Medical Summary Friendly but nervous, allows handling Scan positive BAR, shaking Deep divit center cornea OD that takes up only very faint stain, clear OS, yeasty AU, nndc OP 2/4 tartar PLN wnl H/L reg, no obvious murmur but shaking a lot, ssp, lungs clear/eupenic, no coughing or sneezing ABD snp MSK amb x 4, grade 2/4 medially luxated patella left hind limb Integ generalized severe thicekened tissue/lichenification, alopecia, looks like dried white/grey calcified scabbing, not itchy BCS 3/9 UG f/s, has green tattoo Neuro alert and appropriate A:Dermatitis-looks like demodex vs. less likely sarcoptic mange (not itchy) vs. immune-mediated disease vs. other Desmetocele (deep corneal ulcer) right eye Yeasty otitis externa AU P:Stained right eye-faint uptake in divit center cornea Hold on skip scrape-will start treatment for mites (ivermectin will get demodex and sarcoptic mange) and antibiotics for secondary pyoderma Gave ivermectin 1% solution-2 units PO once today Continue ivermectin and increase to 100 mcg/kg PO SID on 4/2, 150 mcg/kg PO SID on 4/3, 200 mcg/kg PO SID 4/4, 300 mcg/kg PO SID 4/5, 400 mcg/kg PO SID 4/6, 500 mcg/kg PO SID 4/7, 600 mcg/kg PO SID 4/8 until skin has completely healed Monitor for neuro signs/ivermectin toxicity for first 7 days of treatment Start clavamox drops PO BID x 14 days Start TAB opthalmic ointment BID x 14 days, recheck eye in 2 days Start otobiotic BID x 14 days If eyes not getting better, recommend NH so can see ophthalmologist Fair prognosis for desmetocele, consider starting serum Good prognosis for skin but will take time
