Medical Summary Sex: Male intact Scan: Negative Estimated age: 2 years based on body condition and teeth Fleas: No flea dirt. S: Active, friendly, wags tail. Allowed exam with minimal restraint, but pt became tense after first vaccine, and snapped at my fingers for nasal vaccination O: BAR-H, BCS 5/9, MMs pink and moist, CRT <2 sec EENT: No discharge OU, AU, nose. Mild tartar and gingivitis. PLNs: Not enlarged. H/L: NSR, NMA. Eupnic, quiet lung sounds. Abd: Slightly tense, no pain on palpation, no masses palpated M/S/I: Amb x4. No lesions noted. UG: Male intact, testicles soft and symmetrical. A: Mild dental disease in an otherwise healthy intact male young dog Short-term prognosis: Excellent P: 1. Gave 1.1 ml pyrantel PO and Activyl 4-14 during exam 2. Long-term veterinary care should include regular preventative dental cleanings 1088
