Medical Behavior Evaluation ORANGE
Medical Summary Very tense body, wide-eyed, trembling. Growled on auscultation. Muzzled and face covered for exam. Objective BCS 5/9. MMs pink. EENT: Mild serous discharge OU. Clean ears, nose. Oral Exam: Severe tartar and gingival recession. PLN: No enlargements noted H/L: Grade 5/6 systolic murmur with palpable thrill on left side of chest. PMI left side. Resp sinus arrhythmia. Lungs clear, eupnic. ABD: Tense, no masses palpated U/G: Male neutered. MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat. Long nails. CNS: Mentation appropriate – no signs of neurologic abnormalities Assessment 1. Severe heart murmur 2. Severe dental disease Plan 1. Recommend evaluation by cardiologist with chest rads, followed by dental cleaning with rads and extractions as needed 1088
