QARH. clean ears. nuclear sclerosis OU. dental disease, missing most teeth – to be fed soft food only. 5.14lbs; BCS: 2.5/5 thin, has a great appetite; lameness of hindlegs, falls over when attempting to walk, no obvious fractures or wounds
able to extend legs, moves around slowly to eat and urinate – extra bedding provided. calm, allowed all handling. vet exam requested
RE-EAM :
01/17/17 : Geriatric exam.
S/O: QARH. Allows all handling. BCS 2/5
EENT: Dental disease, severe tartar buildup, missing several teeth, nuclear sclerosis, small clear mass just medial to right medial canthus, no ocular or nasal discharge, ears wnl
HL: Clean lung sounds, no murmurs/arrhythmias
ABD: Soft, non-tender, no abnormalities appreciated
INTEG: Appears to have been recently shaved for IVC placement at both front limbs, as well as ventral abdomen; otherwise integ wnl
MS: Weakly ambulatory x 4, reluctact to walk; Grade 3/4 MPL Right stifle, Grade 2/4 MPL Left stifle
UG: Externally normal intact female, small nipples, no spay scar or tattoo seen
A: Geriatric, dental disease, bilateral MPL
P: No treatment indicated at this time. Continue to monitor while at BACC. Good short term prognosis
