Medical Summary Sex: Female Scan: Negative Estimated age: 4 years based on dental tartar and energy level Fleas: No flea dirt. S: Friendly, sits calmly while waiting, watches all activity in the room, soft body, allows full exam and vaccinations, good appetite for all food and treats. Whines when left alone in cage. O: BAR-H, BCS 1-2/9, MMs pink and moist, CRT <2 sec EENT: No discharge OU, AU, nose. Moderate tartar and gingivitis. PLNs: Not enlarged. H/L: NSR, NMA. Eupnic, quiet lung sounds. Abd: Soft, no pain on palpation, no masses palpated M/S/I: Amb x4. Generalized dermatitis on ventral abdomen/mammary glands. Small circle of alopecia over left hip. UG: Female, lactating, all 10 mammary glands are full and soft A: 1. Emaciated 2. Lactating – gave birth recently 3. Dermatitis on ventral abd – R/O urine scald vs. other 4. Moderate dental tartar Short-term prognosis: Fair Current surgery status: Temporary waiver for emaciation, lactation and dermatitis. P: 1. Applied Pet Armor 23-44 lbs, and gave pyrantel 3.4 ml 2. Place third feeding sign – SMALL MEALS ONLY for 6 days (1/4 of normal portion for 2 days, then 1/2 of normal portion for 2 days, then 3/4 of normal portion for 2 days). 3. Cephalexin 250 mg PO BID x10 days for dermatitis; reweigh in 7 days and consider increasing dose 4. Long-term veterinary care should include dental cleaning and appropriate dental hygiene 1088
