Medical Summary Sex: Female Scan: Negative Estimated age: 5 years based on dentition Fleas: No flea dirt noted. Hx: Pt has been shaking her head a lot since the client was given her two days ago S: Cautious, allows exam but nips when I try to listen to her heart. Quiet. Muzzled for vaccines. O: BAR-H, BCS 4.5/9, MMs pink and moist, CRT <2 sec EENT: Crusty ulcerated lesions with waxy discharge AU. No discharge OU, nose. Moderate tartar and gingivitis. PLNs: Not enlarged. H/L: NSR, NMA. Eupnic, quiet lung sounds. Abd: Soft, no pain on palpation, no masses palpated M/S/I: Amb x4. No skin lesions noted. UG: Female, well-developed vulva so likely intact A: 1. Otitis externa 2. Dental disease Short-term prognosis: Good P: 1. Gave 0.8 ml pyrantel and Activyl 4-14 during exam 2. Clean ears q3 days x4 cleanings 3. Otibiotic AU BID x10 days 4. Long-term care should include dental cleaning and X-rays 1088
