Medical Summary DVM Intake Exam Estimated age: reported to be 10 months Microchip noted on Intake? neg History : brought in by neighbor of previous owner; previous owner moved to california and asked neighbor to surrender dog; neighbor has had dog for 4 days and she has been eating well Subjective: BAR, very hungry Observed Behavior – excessive vocalization but will stop when held; overall sweet with no signs of aggression Evidence of Cruelty seen – emaciated Evidence of Trauma seen – RF lameness-neurologic; decreased PLR OS Objective BCS 2/9 EENT: Eyes clear (see neuro), ears clean, no nasal discharge noted Oral Exam: mm pk, sl tacky; CRT <2 sec; moderate tartar build up PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: soft, nonpainful U/G: female MSI: Ambulatory x 4-kyphosis of TL spine; dec hip extension bilat with firm end feel; mild-mod plantigrade stance; mild-mod lateral laxity of R carpus; proprioceptive ataxia unthrifty coat no obvious wounds or injuries CNS: mild proprioceptive ataxia paresis of RF focal seizure/muscle twitching along R side of head/neck/forelimb mentation appropriate – no signs of neurologic abnormalities OD-PLR positive OS-PLR neg dec proprioception on hindlimbs, worse on L side; decreased withdrawal on R side, absent-decreased on L side vocalization-may be behavioral Rectal: Assessment 1. Neurologic dz-r/o trauma vs congenital vs infectious vs metabolic 2.
