Objective: P = WNL R = WNL BCS 5/9 EENT: Right eye buphthalmic with blue-ish pigmentation to sclera and cornea, scleral injection, hyperemic, proptosed third eyelid, left eye WNL, ears clean, no nasal discharge noted
Oral Exam: Moderate dental tartar, pink mm PLN: No enlargements noted H/L: Grade 1-2/6 heart murmur, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Male MSI: Ambulatory x 4, skin free of parasites, ~3-4cm soft mass at lateral right hind limb at region of stifle, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities. Assessment: Geriatric
dental disease Mass RH – R/O lipoma Right eye – R/O chronic glaucoma vs neoplasia vs other, +cherry eye. Low grade heart murmur Plan: Blood work pending. Continue rimadyl 37.5mg PO q12 x 5 days. Rec enucleation of right eye with biospy. Prognosis: Fair SURGERY: Permanent waiver due to age
