DVM Intake Exam; Estimated age: 9+yrs; Microchip noted on Intake? yes; Microchip Number (If Applicable): 985121006289560; History: O/S due to moving to place that does not allow pets; Subjective: Dog is QARH; Observed Behavior – immediately social, wagging tail, allows all handling with minimal restraint; Objective ; T = NA; P = 110; R = wnl; BCS 3/9 (severe muscle atrophy head/face); EENT: Eyes clear, ears wounds/scars at tips of pinna AU, no nasal or ocular discharge noted; Oral Exam: Grade 1-2/4 dental dz; PLN: No enlargements noted; H/L: Muffled heart sounds, CRT < 2, Lung sounds decreased bilaterally; ABD: Non painful, no masses palpated; U/G: male neutered ; MSI: Ambulatory x 4, skin free of parasites, no masses noted, hair coat thin, dry; nails overgrown; CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: not performed; Assessment: Geriatric underlying – Underweight (muscle wasting), dental dz, poor hair coat; Prognosis: Fair ; Plan: Recommend NH placement. LVT pulled blood for CBC/CS/T4. Placed on third feeding. SURGERY: Already neutered
