Medical Summary DVM Intake Exam Estimated age: 3-6 years based on overall appearance Microchip noted on Intake? Scanned POSITIVE History : Adopted from ASPCA in early Sept 2017 – MC registered from Galveston TX. Subjective: Alert, apparently healthy Observed Behavior – Pt was lying on Kuranda bed, watching. His eyes perked up when we walked by – pt got up and came to front of kennel, wagged his tail slowly. Pt allowed leashing and was easy to walk out of the kennel and down the hall. Pt had very tense body and wide eyes. Pt was muzzled (easily) for safety during exam. No signs of aggression during exam but pt was VERY tense, with claws out, tail tucked, crouched on floor. Very difficult to do blood draw bc pt withdrew his legs and jumped up and back to avoid restraint. One tiny short growl at one point – only once. No biting, snarling or snapping. Objective BAR-H, MMs pink and moist, BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Did not perform due to muzzle PLN: No enlargements noted H/L: NSR, NMA, Lungs clear, eupnic ABD: Tense, no masses palpated U/G: Male neutered MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Could not visualize Assessment: Apparently healthy externally Plan: Placement Prognosis: Excellent 1088
