Medical Behavior Evaluation BLUE
Medical Summary DVM Intake Exam Estimated age: neutered male 10 yo Microchip noted on Intake? positive scan – in record History : o surrender 2 dogs – not allowed in housing Subjective: BARH. Observed Behavior – Vocal. Muzzled but allows all handling.  Objective T = P = 120 R = 30 BCS=7/9 EENT: Eyes – marked lenticular changes OU, PLR decreased and incomplete, no discharges, conj/ cornea/ sclera clr. Moderate cherry eye OS. Ears – slightly dirty AS > AD. No nasal discharge noted. Oral Exam: marked tartar covering molars, mod gingivitis, halitosis – full exam not done since muzzled. NSF noted after removing muzzle but unable to do full exam. PLN: No enlargements noted. H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: neutered male MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat. Obese. CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not done. Assessment: 10 yo neutered male Chi. 2.. Cherry eye OS. 3. Dental dz. 4. Obese. 5. Inspiratory increased BVS and effort – r/o due to obesity 6. Decreased vision Plan: Dental prophylaxis recommended but caution due to resp issues. Recom lose wt (3+ lbs) and re-assess lungs after – in 2-4 mos – prior to anesthesia. If still has increased breathing effort, consider xrays. Consider cherry eye sx at same time as dental. Cleaned ears. Prognosis: Fair to good.

Max does not not like to stay in his kennel. He barks in there but stops as soon as i open  his door. Leashed in no time, we take the stairs to the street. Max does up and down  without any problem. He is a great little walker despite his generous weight and does his business on the way. He is a bit shy at first, sitting  in a corner of the yard, his ears  dancing around his head like  chopper’s wings, his mouth  open as he is a little out of breath and his eyes shiny with apprehension . A little pep talk relaxes him and soon enough, he is good to go, tail wagging… He even wants to come on the bench ..for which he needs my help.  Max is a sweet little guy, friendly  and affectionate who comes with  very nice comments from his former owner. He lived happily with a damsel , his age but ten times his size and who is with us too, Duchess. He was initially timid but friendly to strangers and children,  free of handling issues but anxious although not destructive when left alone. Max would love  to be once again a cherished pet and give out  all the love his tiny Chihuahua’s heart holds. Come and meet him soon at the Manhattan Care Center and take him home…
PROFILE :
Basic information: Max is a 10 yr old dog who came in with his house sibling Duchess (A1125580). He was gotten from a pet store and was kept in the same home since he was just a few months old. Unfortunately due to landlord problems he could not be kept. He was last brought to the vet last year for his yearly vaccinations.
Socialization: Around strangers and children Max is described fearful, relaxed and respectful and plays gentle with them. Max has been socialized with a house dog sibling and is described as relaxed, playful and also plays gentle. He has never been socialized with cats and has no history of ever biting another person or animal.
Behavior: Max will occasionally use the bathroom indoors, becomes anxious when he is left alone, barks a lot and during loud storms or fireworks he is unbothered. He enjoys being bathed and being brushed and isn’t bothered by having his food or toys touched, being held or restrained, being disturbed while asleep, has never had his nails trimmed and will bark continuously when unfamiliar people approach his owner or home.
For a New Family to Know: Max is described as friendly, affectionate, shy, mellow, independent, fearful and pushy. He has a low activity level, will follow you around when home with you, likes to play with stuffed animals and squeak toys. He has been kept mostly indoors, prefers to sleep on his own bed, ate Pedigree dry food 3x a day, seems to be house trained and has accidents about 2x a week and when he is left alone at home he will bark and whine. He has never been crate trained, likes a to be walked at least 2-3x a day, doesn’t pull on leash and off leash will wander but comes when he is called.
Intake: Upon intake Max allowed minimal handling. He would bark continuously at counselor whenever he was approached and when attempting to collar he would hide and move away. Minimal handling was performed to reduce stress.
