Medical Summary DVM Intake Exam Estimated age: 5Y Microchip noted on Intake? no History : O/S due to his reactive behavior towards people Subjective: Dog is BARH. Observed Behavior – Handler was able to get dog out of run, jumped on his arm and sniffed, but no attempt to bite. Once in dog exam, was able to get on scale for weight, but dog was showing signs of fear (tail tucked, whale eyed). When LVT went to muzzle dog lifted lip and then immediately snapped making contact with her hand (did not break skin). HW test and MC forfeited. Dog placed in squeeze gait to vaccinate. Objective T = NA P = NA R = NA BCS 5/9 EENT: appear grossly normal Oral Exam: NE PLN: NE H/L: NE ABD: NE U/G: both testicles clearly visible MSI: Ambulatory x 4, coat appears healthy and free of parasites CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: Apparently healthy Plan: Recommend rescue placement with experienced trainer or humane euthanasia due to behavior issues. Prognosis: Good SURGERY: comment says do not schedule for S/N
