Active, attention seeking, allows all handling Objective P = WNL R = WNL BCS 4/9 EENT: Nuclear sclerosis, ears clean, no nasal discharge noted Oral Exam: Missing most (all?) teeth, gingivitis PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated, tense U/G: Male MSI: Ambulatory x 4, skin free of parasites, no masses noted, unkempt hair coat, ~1cm round raised dermal mass, multiple sebaceous cysts CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Geriatric, dental disease, mass, multiple sebaceous cysts Plan: Rec CBC/chem for tomorrow. Continue to monitor. No treatment indicated at this time Prognosis: Good SURGERY: Permanent waiver due to age
