My name is MAX. My Animal ID # is A1075301.
I am a neutered male white and black shih tzu and lhasa apso mix. The shelter thinks I am about 2 YEARS old.

I came in the shelter as a OWNER SUR on 05/27/2016 from NY 11385, owner surrender reason stated was CHILDCONFL.

MOST RECENT MEDICAL INFORMATION AND WEIGHT
05/28/2016 Exam Type VACCINATE – Medical Rating is 1 – NORMAL , Behavior Rating is NONE, Weight 26.0 LBS.

all vaccines given by 0844 pyrantel 2.6ml po activyl 1ml topically

05/27/2016 PET PROFILE MEMO
05/27/16 14:51 BASIC INFORMATION: Max is a white/black Shih Tzu/ Lhasa Apso mix, 2yrs old male. Max was given to the owner as a gift & has no known health issues. Max was surrendered to the shelter due to child conflict & behavior. DOCIALIZATION: Max is not friendly and social towards strangers. He will lunge growl & bark when a stranger comes near him. Max has resided with small children & has snipped at one of the children months prior to surrender. He has not bitten within the past 10 days. Max is only tolerant of owners presnce and is okay when the owner pets him. Max has not resided with other pets and when he is taken out he will growl and bark at other dogs, he does not engage in play with other dogs. Max has not been exposed to cats or other small animals. BEHAVIOR: Max will growl and attempt to bite when he is eating and owner or anyone comes near him. He is also this way when owner or someone else is trying to take an object from his mouth. Max is tolerant of getting groomed by owner only. He will bark when someone unfamiliar is outside the home or coming near the owner. FOR A NEW FAMILY TO KNOW: Max is best described as a fearful & shy dog. His activity level is medium & he enjoys going for walks. When owner comes home, he will follow owner around. Max enjoys playing with balls & will run around with his toy. Max would do best is a single person home. Max has been kept mostly indoors & enjoys sleeping with on dog bed. Max enjoys eating dry food from any brand. Max is house trained & has not been crate trained. When he is left home alone he will become destructive and destroy house hold items. Max is not familiar with any commands. He is house trained & pulls slightly when he is walked. Max will run away when left off the leash. BEHAVIOR UPON INTAKE: Max was not tolerant of counselors presence. He growled and barked at the counselor & allowed little to no handling.

05/31/2016 BEHAVIOR EVALUATION – NH ONLY
Exam Type BEHAVIOR
Helper dog: A1074746 Max previously lived with adults and small children. The owner stated Max is not social towards strangers, he will lunge growl and bark when a stranger comes near him. The owner mentioned Max has snipped at one of the children. it was reported by the owner Max will growl and attempt to bite when he is eating or when someone comes near him. The owner describes Max as a fearful and shy, he is house trained, has a medium activity level and becomes destructive in the home. At intake it was reported Max was not tolerant he growled and bark at the counselor, no handling was conducted. During SAFER Max came into the assessment room tense and fearful, he was not interested with the assessor. Not all handling items were able to be conducted as Max bared his teeth, growled and lunged at the assessor when the assessor comes near him. He displayed no interest in the toy item during this portion of the assessment. When on leash for a dog interaction with another dog on leash Max does not approach, but instead turns his body to the side and exits. In light of this behavior we feel that placement with a rescue who can provide positive reinforcement-based behavior modification is the best option at this time. Toy 1: No interest. Dog-dog: 2. Dog does not approach the helper dog. Turns body to side in relation to other dog, or exits.

05/29/2016 GROUP BEHAVIOR EVALUATION
Exam Type GROUP BEHAVIOR
According to Max’s previous owner, Max has not resided with other pets and when he is taken out he will growl and bark at other dogs. He does not engage in play with other dogs. An attempt was made to remove Max from his kennel, but due to his reactivity he was not able to be removed at this time.

05/28/2016 INITIAL PHYSICAL EXAM
Medical rating was 1 – NORMAL , behavior rating was NONE
scan neg ~2yrs, neutered limited exam due to temperment — difficult to get out of cage, tried to bite, lunge teeth clean, eyes clear, no ocular or nasal discharge integ wnl; ambulatory x 4 a: apparently healthy p: continue to monitor while at bacc. excellent prognosis
