Medical Summary Scan negative M; 3 YO; 59 LB Was extremely freindly during exam; seeking attention and reaching out with paw; however, there is a large wound on his back between the shoulders, and when touched in this area he snarls and gives sideways focused glances, likely due to pain Owner reports dog was hit by a car a week ago and has not been to a veterinarian or had any treatment since the incident There is a large wound on the back, just about between the shoulders, predominently covered with a large scab; tail appears to be crooked downward Suspect pain in the rib/chest/back area due to behavior change when touched anywhere in the vicinity of the wound No limping obs; all cp’s appear WNL at this time BARH

RE-EXAM :
03/14/16 : VC: Reported HBC last week, check scabbed wound between shoulders, asses for source of pain. S/O: BARH. MM pink, moist. Moderate yellow discharge AU, lichenificiation of pinna and vertical canal AU. Cardiothoracic auscultation WNL. Abdominal palpation WNL>. Large scabbed wound approx 10cm lond 4-5cm wide on cranial dorsum. Sedated w/ 1.3ml hydromorphone and 1.3ml dexmedetomidine IV for thoracic and abdominal radiographs. Thoracic radiographs: Lungs field show mild bronchial pattern, fracture located at 8th sternebrae, costochondral junction remodling present. NOSF. Abdominal radiographs: WNL. Clipped hair around wound, soaked scab off w/ dilute chlorhexidine and cleaned. Applied SSD topical. Cleaned ears, applied mometamax AU. A: Wound, fractured one caudal sternebrae, otitis externa; P: Cephalexin, Rimadyl, Mometamax AU SID x 7days. Minor rib/sternebrae fracture will self-resolve, pain control only. Prognosis: Excellent

Basic Information:
Max is a three year old Basset Hound/golden retriever mix who was with his previous owner for 3 months.
Socialization:
Around strangers Max is friendly, outgoing and plays gently with adults. Max has lived with chidlren ages 2 and 3 years old; around them Max is relaxed and plays gently. Max has spent time with dogs of various breeds and sizes at the dog park; around them Max is relaxed and plays gently with them. Max has never been around cats so it is unknwon how he will react to them. Max has never bitten another person or animal.
Behavior:
Max is not bothered by thunderstorms or fireworks. If someone touches Max’s food or bowl while he is eating, take a bone away or take a toy or other obect away Max is friendly toward them. If someone pushes/pulls Max off of the furniture or disturb him while he is sleeping he is friendly toward them. Max is friendly toward people if they give him a bath, trim his nails, brush his coat, or restrain him. If an unfamiliar person approaches Max’s home or family Max is friendly toward them.
For a New Family to Know:
Max is described as: friendly, affectionate, playful, confident, excitable and independant with a medium activity level. Max’s favorite activity is running and tug-o-war. When Max is at home he will follow around his owner and likes to play with balls, stuffed toy and squeaky toys. Max has been kept indoors only and has been fed both wet and dry food on a freeding schedule. Max is house trained and rarely has any accidents. When left alone in the house or yard Max is well behaved. Max has not been crate trainedand is used to brisk walks, playimg in the yard and off leash dog parks.
Intake Behavior:
Max allowed staff to collar, scan for a microchip and take ictures with no signs of aggression. Max was very friendly during his intake and greeted staff with a wagging tail and a loose body.
