DVM Intake Exam; Estimated age:8-11 y; Microchip noted on Intake?n; History :stray; Subjective:thin, ate ravenously; Observed Behavior -allows full exam, curious; Objective ; T =; P =60; R =wnl; BCS 3/9; EENT: ou sclerosis, ears clean, no nasal or ocular discharge noted; Oral Exam:moderate tartar; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G:neutered male; MSI: Ambulatory x 4, skin free of parasites, wart like mass on dorsum; CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: Assessment: generally healthy, small mass on back, thin; Prognosis:good; Plan:no treatment. monitor weight

Basic Information: Max is an 11 year old neutered male that has no previous or current medical issues that the owner is aware of. Owner had Max for 11 years but had to surrender due to moving
Previously lived with: 2 Adults, 1 Child
How is this dog around strangers? Around strangers, Max will bark. Owner stated that he is usually shy around new people for several hours but once comfortable, he plays gently
How is this dog around children? Max has been around a 6 year old child and was always relaxed and playful around the child
How is this dog around other dogs? Owner had another male dog in the home at one point and stated that Max was always relaxed and respectful around the other animal but the other dog would bark at him
How is this dog around cats? Information is unknown
Resource guarding: Max is not bothered if someone takes away his food, treats or toys

Other Notes: Max is not bothered when someone restrains him, pushes him off the couch, gives him a bath, brushes his coat and trims his nails. when an unfamiliar person will approach the home or owner, Max will bark

Medical Notes: Max does not have any known medical concerns
For a New Family to Know: Max is described as friendly, affectionate, playful and excitable. He has a medium activity level and likes to play with all types of toys. In the home, Max liked to stay in his favorite spot. He was fed dry food only. Max was kept mostly indoors and is not house trained. He would urinate in the home several times a day. When left alone in the home, Max would go potty indoors. Max is crate train and would spend several hours a day in a crate. He knows the cues sit and come. For exercise, he was taken on brisk walks on the leash. When on leash, Max tends to pull very hard
Behavior Notes: Behavior during intake: Max had a tense body and was barking non stop during intake. Counselor was able to scan for a microchip, collar and take a picture without any issues.
