Objective ; BCS 4/9 ; EENT: Eyes clear, ears clean, no nasal or ocular discharge noted; Oral Exam: dental calc 4/5; periodontal dz 3/5 ; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: testicles within scrotum; smooth, symmetrical. MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat; grade 4 patellar luxation, left. grade3 patellar luxation R. CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: normal on external exam. Assessment; patellar luxation; dental calculus ; periodontal disease ; underweight. Prognosis: good ; Plan: ok for neuter. recommend dental cleaning and daily dental care once placed. SURGERY: Okay for surgery
