Medical Behavior Evaluation BLUE
Medical Summary scan: neg; sex: female; est age: 12-15yrs; S/O: Dog is dull/depressed; Licked and/or ate a small amount of canned food offered; BCS 2-3/9; EENT – OD sunken and covered by conjunctiva, cataract OS; Grade 3/4 dental dz; Heart/lungs – heart muffled on right side, grade II/VI murmur left side. Dog is weak but ambulatory, CPs wnl; Coat is unkempt, there are several nodular masses and open wounds right shoulder; healing laceration medial aspect right hind; appears to have old wound right flank that has scarred together (dog allowed all handling up until clipping/cleaning of flank wound when she whipped around and attempted to bite). A: Geriatric underlying – old wounds, cataracts, dental dz. P: Inform NH to send plea with deadline of 4pm 5/31/16, unless dog becomes brighter and starts eating overnight can stay stray. Keep in medical to monitor, recheck tomorrow. PROGNOSIS: Poor-Guarded
