Margie was unable to walk, and allowed clients to pick her up and place her into their car. O-QAR, weak, icteric and slightly pale mm, CRT~2 sec, +/- hydrated EENT-CAU, old scarred aural hematoma AS, COU, no nasal d/c PLN-all palpated wnl CV_HR=100, no m/a, fsp Resp-RR=20, clear GI-negative oral, distended abdomen with fluid wave GU-FI MSI-BCS 2.5/5, non-ambulatory/weak, unable to put weight on feet even with assistance, edema of distal hock bilaterally (L>R), intidigital between digits III and IV of LHL-moist skin lesion, raised skin/mass in lateral aspect of L elbow, skin abrasion in left flank Neuro-QAR, normal cranial nerves, not seen on walk, no CP deficits A-3-4 yo FI Am Staff 1. icteric R/O 2. ascites vs uroabdomen vs other 3. weak 4. skin lesions (elbow mass, skin abrasion, moist interdigital skin lesion) P-abdominocentesis-straw colored fluid CBC-moderate microcytic macrochromic anemia (26%), leukocytosis (19.37 K/ul) with neutrophilia (17.19 k/ul) and eosinopenia (0.02 k/ul), adequate platelets Chemistry-eleavted BUN (78 mg/dl), hyperphosphatemia (7.4 mg/ml), elevated liver enzymes (ALT>1000 U/L, ALKP 1228 U/L, GGT 47 U/L, tbil 12.7 mg/dl), electrolyte abnormalities (Na 132 mmol/L, K 2.4 mmol/L, Cl 97 mmol/L) abdominal fluid analysis-usg=1.028, similar chemistry profile to peripheral blood (BUN 79 mg/dl, Phos 7.2 mg/dl, ALT 467 U/L, ALKP 482 U/L, GGT 33 U/L, tbil 6.8 mg/dl, Na 135 mmol/L, K 2.5 mmol/L, Cl 98 mmol/L), hypercalcemia (7.6 mg/dl) Recommend transfer to 24 hour facility for treatment. Prognosis guarded.

Behavior during Intake
Although, injured Margie was friendly during intake. She attempted to interact when spoken to, but was too weak. She allowed petting and collaring and showed no signs of aggression during intake.
