PLN: No enlargements noted; H/L: NSR, 4/6 holosystolic heart murmur, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: female spayed, no mgts or dc; MSI: Ambulatory x 4,thin, skin free of parasites, no masses noted, healthy hair coat. overgrown nails. CNS: Mentation appropriate – no signs of neurologic abnormalities; Assessment: heart murmur; oligodontia/periodontal disease; Prognosis: good; Plan: start on doxycycline for periodontal disease; recommend cardiac evaluation; recommend periodontal therapy if possible; continue to monitor at ACC; SURGERY: already altered
