Medical Summary Estimated age: 2 yo Microchip noted on Intake: Negative Stray, BAR Evidence of Cruelty seen – Yes Evidence of Trauma seen – None Hydrated; T =Not taken P = 100 R = 24 BCS 5/9 EENT: Eyes clear, ears dirty, (brown discharge) no nasal discharge noted Oral Exam: No dental tartar, all permanent teeth are present, MM Pink and moist, PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Intact female, distended (swollne) nipples (seems that recently had puppies) MSI: Ambulatory x 4, skin free of parasites, no masses noted, dirty hair coat, several sacars and some dermatitis aroun neck CNS: Mentation appropriate – no signs of neurologic abnormalities (full neurologic exam not performed) Rectal: Not performed Assessment 1.
