DVM Intake Exam; Estimated age:8-10y; Microchip noted on Intake?n; History :stray; Subjective: Observed Behavior -scared, allows exam; Objective ; T =; P =60; R =wnl; BCS 5/9; EENT: ou sclerosis, ears clean, no nasal or ocular discharge noted; Oral Exam:severe dental disease; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: no ss; MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat; CNS: Mentation appropriate – no signs of neurologic abnormalities; Assessment: severe dental disease; Prognosis:good; Plan:dental ohe in future; SURGERY:Permanent waiver due to age

Basic Information:  Maggie was brought in as a stray so her history is unknown at this time. She appears to be a 7-8 year old, small mixed breed female.

Behavior Notes:
Maggie was very friendly and affectionate during intake. She jumped up on counselors lap and was nuzzling into her shoulder. She enjoyed being held and was wagging her tail.

Date of assessment:16-May-2018
Look:2. Dog pulls out of Assessor’s hands each time without settling during three repetitions.
Sensitivity:1. Dog stands still and accepts the touch, eyes are averted, and tail is in neutral position with a relaxed body posture. Dog’s mouth is likely closed for at least a portion of the assessment item.
Tag:1. Dog follows at the end of the leash, body soft.
Paw squeeze 1:1. Dog gently pulls back his/her paw.
Paw squeeze 2:1. Dog gently pulls back his/her paw.
Toy:1. No interest.
Summary:Maggie displayed a soft body posture, soft eyes, and relaxed ears. She was calm throughout the assessment; playful during tag, but showed no interest in the toy item.
Summary (1):Maggie was surrender as a stray so her past behavior with other dogs is unknown.
5/15-16: When off leash at the Care Centers, Maggie greets a group of small male and female dogs with a soft posture. She is social and explores the yard.

Summary:Maggie was very friendly and affectionate during intake.

Summary:Maggie was a bit nervous, but allowed handling.

ENERGY LEVEL:Maggie displayed a low-medium energy. Playful during tag, and gentle. Soft body and allows handling. She will need daily physical and mental stimulation to keep her engaged and exercised.

BEHAVIOR DETERMINATION:AVERAGE (suitable for an adopter with an average amount of dog experience)
