Medical Summary DVM Intake Exam Estimated age: 6-8 years Microchip noted on Intake? negative History : stray, found outside yesterday. Finder offered food which she threw up Subjective: QAR and 5% dehydrated. Presented very lethargic and shaking. Used a sling to walk her to medical Observed Behavior – Too lethargic to react, shaking all over Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = wnl R = wnl BCS 3/9 EENT: mucoid ocular d/c ou. Serous nasal d/c ou. Growth on lower eyelid OS. Oral Exam: adult dentiton with moderate dental disease, gums are brick red and CRT <2s PLN: No enlargements noted H/L: NSR, Grade 4/6 HM, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: FI, enlarged hooded vulva with purulent d/c MSI: Ambulatory x 4 but very weak, MCS 2/3, skin free of parasites, no masses noted, healthy hair coat. Small growth on body (fibromas), ~2cm growth on left lateral thigh CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Purulent vaginal d/c r/o pyometra vs UTI Lethargic/Shaking Dehydrated URI Grade 4/6 HM Dental disease Fibromas Growth on lower left eyelid Grown on left lateral thigh Thin BCS r/o neoplasia vs infection vs other underlying condition Renal failure (azotemic, hypocalcemia, hyperphosphatemia) Anemia Plan: CTM while at BACC TAB ointment OU Baytril 10mg/kg PO SID x10d 2 view AXR-nsf CBC: leukocytosis 16.99 (5.05-16.76), neutrophilia 15.17 (2.95-11.64), non-regenerative anemia 26.9% Chemistry: Azotemia, BUN >130 (7-27), Creat >13.6 (0.5-1.8), hypocalcemia 6.2 (7.9-12), hyperglycemia 163 (74-143), hyperphosphatemia >16.1 (2.5-6.8) Prognosis: grave due to CKD and renal failure SURGERY: Temporary waiver due to sickness
