Medical Summary DVM Intake Exam 7/13/17 Estimated age: ~10yrs, male Microchip noted on Intake? No History : Stray Subjective: Observed Behavior – BARH. Allows all handling Objective P = WNL R = WNL BCS 4/9 EENT: Nuclear sclerosis, ears clean, no nasal discharge noted Oral Exam: Severe dental tartar/gingivitis, halitosis PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Male, right testicle very firm, left testicle soft and atrophied MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Geriatric, irregular testicle, dental disease Plan: Continue to monitor while at BACC. Rec dental cleaning +/- extractions and neuter with placement Prognosis: Good SURGERY: Permanent waiver due to age
