KNOWN HISTORY: 2/8/17 Neutered, Stray (length of time in previous home) Bite history: Yes, bit the owners mother on the stomach, and then bit the owner and his son on the hand when they were trying to pull him away. Severity of the bites is not known. SAFER ASSESSMENT: 2/15/17 Look: 1. Dog’s eyes are averted, with tail wagging and ears back. He allows head to be held loosely in Assessor’s cupped hands. Sensitivity: 1. Dog leans into the Assessor, eyes soft or squinty, soft and loose body, open mouth. Tag: 1. Dog assumes play position and joins the game. Squeeze 1: 1. Dog does not respond at all for three seconds. Squeeze 2: 1. Dog does not respond at all for three seconds. Toy: 1. Dog settles close, keeps a firm grip and is loose and wiggly. He does not place his body between you and the toy. Summary: Lucky displayed no concerning behaviors on his assessment, he was social throughout. DOG-DOG INTERACTION ASSESSMENT: Summary PLAYGROUP: Summary MEDICAL BEHAVIOR: Date of Initial ENERGY LEVEL: We have no history on Lucky so we cannot be certain of his behavior in a home environment. However, he is a young, enthusiastic, social dog who will need daily mental and physical activity to keep him engaged and exercised. We recommend long-lasting chews, food puzzles, and hide-and-seek games, in additional to physical exercise, to positively direct his energy and enthusiasm. RECOMMENDATIONS: New Hope Only _X_No children (under 13): Due to Lucky’s bite history, we recommend an adult only home. He has shown restraint in the care center and has not demonstrated any warnings of aggression, but as the circumstances and severity of the bites are unknown we recommend an adult only home. _X_Place with a New Hope partner: Due to Lucky’s multiple bite history, we recommend he be placed with a New Hope partner who can provide positive reinforcement, force free training behavior modification under the guidance of an experienced trainer or behaviorist. His behavior should be reevaluated once in an experienced, adult only home. Potential challenges: _X_Multiple-bite history/risk of future aggression (Lucky has a multiple bite history, the circumstances and severity of which are not known. Lucky needs careful behavior modification and management to prevent future bites. We recommend only force-free, reward-based training methods for Lucky.)

02/16/17 16:38 s/o: on VC for wound on top of head and CIRDC monitoring clean nonerythematous area of alopecia approx 0.5cm x 2cm on top of head no nasal discharge, no s/c, good energy, takes treats a: no CIRDC signs superficial wound p: cwct for otitis no tx needed for wound on head VC: Check ears. Hx: Severe otitis esp AD on intake. Hit top of head on part of cage on 2/13. S/O) BARH. Ears: Stenosis and hyperkeratinization AU – worse on the R. But no discharges. Linear 2 cm wound on top of head healing well. Noted moderate amount SND and sneeze after putting on muzzle and some cough when pulling on leash. But when muzzle taken off and dog relaxed, no c/s/ nasal discharge. A) Otitis externa improved. P) CWPT. Monitor for CIRDC.
