Lucky 8982, 8 Years Old, Small Black and Tan Mix, Neutered, 19 lbs, Owner Surrender 10/06/17
Medical:[DVM Intake]; Estimated age: 8 years Microchip noted on Intake? no History : surrender and has history of seizures as per owner; Subjective: BAR, no evidence of any neurologic disorders or abnormalities; Observed Behavior – timid and shy however allowed all handling; Objective; T =; P = 122 ; R = 34 ; BCS= 7/9; EENT: Eyes clear, ears clean, no nasal discharge noted; Oral Exam: mild plaque and tarter; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated, pendulous abd. ; U/G: no testicles palpable; MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat; CNS: mentation appropriate – no signs of neurologic; abnormalities; Rectal: not performed; Assessment – R/O intracranial vs extracranial causes of seizures; Prognosis: Guarded; Plan: CBC/CHEM/T-4; Midazolam 5mg/ml – gave 0.37ml IM
Lucky has a reported history of seizures that began in 2016. He was surrendered today because unfortunately his owner didn’t feel they could manage his medical needs long term, and he reportedly had a seizure around 6:30am this morning, as well as one shortly after his initial medical exam. Lucky needs placement by 6pm today and will need care at a 24 hour vet immediately.
