DVM Intake Exam; Estimated age: 12 years; Microchip noted on Intake? negative; History : o/s-o reports HM; Subjective: BARH; Observed Behavior – very friendly and allowed all handling and exam; Objective ; P =wnl; R =wnl; BCS=4/9; EENT: Eyes have thick musoic d/c with corneal scarring OU, ears clean, no nasal discharge noted; Oral Exam: moderate to severe dental disease with prognathism; PLN: No enlargements noted; H/L: NSR, Grade 5/6 HM, CRT < 2, Lungs clear, eupneic; ABD: Non painful, no masses palpated; U/G: MN; MSI: Ambulatory x 4, skin free of parasites, small BB sized dermal mass on right shoulder, greasy hair coat; CNS: Mentation appropriate – no signs of neurologic abnormalities; Assessment: Geriatric; KCS; Grade 5/6 HM; Moderate to severe dental disease; Mass on right shoulder-BB sized; Prognosis: Fair to poor-will need longterm treatment; Plan: CTM while at BACC; STT-0 OU @ 30s; Fluorescein stain negative OU; Start optimmune OU BID until ophthalmology consult (first dose given) Start erythromycin OU BID x7d until 7/5 then recheck eyes; Rec ophthalmology consult; Rec echocardiogram for HM; Rec geriatric BW with placement; Rec histopath of mass on right shoulder; SURGERY: neutered

Basic Information:Lucky is a small grey neutered dog that was surrendered to the ACC due to the fact his owner no longer had time to care for him. the owner stated that she got Lucky from a relative and she has had him since he was 8 weeks old.

How is this dog around strangers?Owner stated that when around strangers Lucky is friendly and outgoing. When playing with adults he is gentle.
How is this dog around children?Lucky has spent time in the home around children all ages and he i relaxed and affectionate. when playing with children he is gentle.
How is this dog around other dogs?Lucky has spent time in the home with other small dogs and he is relaxed and affectionate. When playing with other dogs Lucky is gentle.
How is this dog around cats?Lucky has never spent time in the home around cats so it is unknown how he will react.
Resource guarding:Lucky is friendly if someone was to touch his food treats or toys. If someone unfamiliar approaches his house or family member he is not bothered.

Other Notes:Owner stated that overall Lucky behaves well. During loud noises and fireworks he is not bothered. Lucky will tolerate being bathed brushed and having his nails trimmed. he is not bothered if he is held,restrained, pushed off the couch or disturbed while he sleeps.

For a New Family to Know: Owner described Lucky as friendly and mellow. When at home he likes to be in his favorite spot. He does not care to play with toys since he has gotten older. Lucky has been kept mostly indoors and eats dry dog food. When using the bathroom outside he will go on the grass or cement. When left alone in the home he might urinate on the floor. When left alone in the yard he is well behaved. He has never been crated so it is unknown how he will react. Lucky knows how to sit and come when called. He is used to slow walks on the leash and playing in the yard. On the leash he does not pull. Off the leash Lucky will stay by your side.
Behavior Notes:During intake Lucky allowed the counselor to collar and take his picture with no problems.

Look: 1. Dog’s eyes are averted, ears are back, tail is down, relaxed body posture. Dog allows head to be held loosely in Assessor’s cupped hands.
Sensitivity: 1. Dog stands still and accepts the touch, eyes are averted, and tail is in neutral position with a relaxed body posture. Dog’s mouth is likely closed for at least a portion of the assessment item.

6/28-7/1: When off leash at the Care Centers, Lucky greets a group of small male and female dogs with a relax posture. He mostly keeps to himself while exploring the yard.
