Medical Summary Estimated age: 8 years old Microchip noted on Intake? scan negative History : stray, history of being hit by a car, was examined by a vet at another hospitla and no injuries were found Subjective: Observed Behavior – friendly Evidence of Trauma seen – superficial abrasion Objective BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: severe dental calc and gingivitis PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: FI, prominent vulva and mammary glands, no vaginal discharge MSI: Ambulatory x 4, skin free of parasites, no masses noted, superficial abrasion at lateral thigh CNS: mentation appropriate – no signs of neurologic abnormalities Assessment severe dental disease Plan recommend dental Prognosis: good SURGERY: Permanent waiver at ACC due to age
