[LVT Intake Exam]:  Microchip Scan: negative, placed 985113001793275 Observed Behavior: allows all handling but tense, seems “sleepy” in kennel (can be roused but chooses not to interact with handler, shut down but not lethargic). Sex: intact male. Estimated Age: reported 15y  Subjective: surrendered, no history. Eyes: slight opacity. Ears: clean Oral Exam: moderate to heavy staining. Heart: possible abnormal sounds. Lungs: WNL. Abdomen: WNL. Musculoskeletal: underweight or emaciated 2.5-3/9 BCS. Mentation: QARH  Preliminary Assessment: geriatric with possible “old age” complications Plan: DVM intake
