Medical Summary 02/16/16 16:06 Scan : negative Sex: Female Approx 3-5 yr S/O: BAR, no ocular or nasal discharge, ears ENL. Dental disease 1/4, slab fracture of PM 4, right side. Grade 3/6 left apical systolic murmur, grade 2/6 right apical systolic murmur. BCS 3/9, muscle wasting most prominent over axial skeleton. Very long front nails. Clipped nails Nervous during exam, ok with slow gentle handling A: Underweight, mild dental disease w/ tooth fracture, heart murmur, suspect cardiac vlave insufficiency. P: Will start 3x/day feeding and weekly weight monitoring. Recommend NH plea for possible placement w/ rescue group. Recommend echocardiogram to asses cardiac function, dental evaluation and likely extraction of right PM4. Will be permanently waived from OHE at ACC due to heart murmur. PROGNOSIS: Good
