Large mat under the right ear. Oral Exam: Severe tartar and gingival recession PLN: No enlargements noted in mandibular or popliteal LNs. H/L: Grade 4/6 systolic heart murmur, no thrill. NSR. Lungs clear, eupnic. ABD: Tense. U/G: Female, developed vulva. Small lumpy irregular masses associated with the right mammary chain – masses 1-3 cm long. MSI: Ambulatory x 4. Mats encircling the lower legs. Very long nails. CNS: Mentation appropriate – no signs of neurologic abnormalities Assessment 1. Geriatric 2. Severe dental disease 3. Heart murmur 4. Mammary masses – R/O benign vs. malignant 5. Mats and long nails Plan 1. Sign up for grooming and nail trim 2. Run CBC/chemistry 3. Recommend chest rads, possibly echocardiogram, cardiac medication as indicated 4. Recommend FNA of masses 5. Consider dental cleaning with rads and extractions – this will greatly improve pt comfort and oral hygiene, but it may be difficult given cardiac condition Prognosis: Fair-poor SURGERY: Permanent waiver due to geriatric and heart murmur. 1088

PROFILE :

Basic: Lily is a 16 year old female Shih-Tzu. Her owner got her as a gift from someone else and has had her for about 16 years. She has to surrender her because she is moving to another state and her new apartment does not allow pets. Lily does not have any illnesses and has not had any injuries. She has a skin allergy on her inner thigh and will itch it a lot. She has not been to the vet in a while.

Socialization: Lily is friendly around strangers. She has grown up with children, now they are ages 4 ans 11 and with them and their friends she is relaxed and playful. She plays gently with adults and children. She has not spent much time with other dogs or cats or pets. She does not have a bite history.

Behavior: Lily does not get walked and uses a training pad at home but will have accidents off the pad. She will also whine and bark when left alone. She is nervous and hides during storms and loud noises. She isn’t bothered when someone touches her food while eating, takes away a treat or a toy, pushes her off furniture, holds her, disturbs her while sleeping, or when someone unfamiliar approaches her owner. She struggles when getting a bath, getting brushed, or getting a nail trim. She will bark when someone unfamiliar approaches her home.

For New Family: Lily is described as friendly, affectionate and playful. She is also mellow and her energy level is low. Her owner loves how great she is with her children. Lily loves to sleep and play with her ball, she also will fetch! She follows her owner around the house and will sleep on a dog bed or in her owner’s bed or anywhere in the home. She has been kept mostly indoors for the majority of her life, and has not been walked outside as she has gotten older because the stairs were a problem for her. She eats both wet and dry food 2xday. She is housetrained and uses the training pad at home each time she goes. If outside, she goes on the cement. She will bark and whine when she is left alone. For exercise she just likes to walk around the apartment. She doesn’t pull on the leash and will stay very close to you when off leash.

During intake Lily was bright and alert and sniffing everywhere. She allowed all handling and collaring.

BEHAVIOR :

Unaltered female, owner surrender (lived in previous home for sixteen years)

Previously lived with: Children, ages 4 and 11- they grew up together

Behavior toward children: Relaxed, playful, and gentle with all children

Housetrained: Partially, uses wee-wee pad mostly, may miss and have accidents off the pad

Energy level/descriptors: Friendly, affectionate, playful, and mellow with low energy

Other notes: Owner reports Lily does not get walked anymore due to having trouble up and down the stairs. She may whine and bark when left home alone. Lily hides during storms and loud noises. She struggles while being bathed, brushed, or groomed.

SAFER ASSESSMENT:

Look: 1. Dog’s eyes are averted. Her ears are back, her tail is down, and she has a relaxed body posture. Dog allows head to be held loosely in Assessor’s cupped hands.

Sensitivity: 1. Dog stands still and accepts the touch, her eyes are averted, and her tail is in neutral position with relaxed body posture, mouth open

Tag: 1. Follows at end of leash, body soft

Squeeze 1 / 2: 1. Dog does not respond at all for three seconds. Her eyes are averted and her ears are relaxed

Toy 1. No interest. This may be due to environment as owner reports Lily enjoys playing with balls and games of fetch

Summary:   Lily displayed no concerning behavior during her assessment.

PLAYGROUP:

5/13: When off leash with other dogs, Lily greets politely. She is forward with her greetings and continues to sniff the other dogs even after they offer correction.

MEDICAL BEHAVIOR:

During her initial medical exam, Lily allowed all handling.

ENERGY LEVEL:

Her owner  describes Lily as having a low energy level which matches what we have seen in the care center.

RECOMMENDATIONS: Average (suitable for an adopter with an average amount of dog experience)
