DVM Intake Exam; Estimated age:14 reported; Microchip noted on Intake? n; History : o surrender; Subjective: Observed Behavior -calm, allows exam; Objective ; T =; P =80; R =pant; BCS 6/9; EENT: OU crusty dc and corneal scarring–eyes are moist (kcs unlikely) OD cherry eye, ears clean, no nasal or ocular discharge noted; Oral Exam: advanced dental disease; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: SS felt; MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat; CNS: Mentation appropriate – no signs of neurologic abnormalities; Assessment: chronic eye disease, dental dz; Prognosis:good; Plan:tab ou, consider schirmer tear test, consider dental and cherry eye removal

BEHAVIOR:

11/18: When introduced off leash to other dogs, Lily keeps mostly to herself.
