Medical Summary 12/27/16 Microchip scan pos AVID #106108770 (note: microchip is located on right ventrolateral thorax) O:BARH. mm=pink, moist, CRT<2 s. Friendly, easy to handle. ORAL: Severe calculus, gingivitis. EENT: OU – central black opacities. No discharge or scleral injection. H/L: Marked upper resp. stertor. Mildly increased bronchovesicular sounds but difficult to fully assess due to upper airway noise. No murmur ausculted. ABD/UG: Female, no spay scar evident. Palpation WNL. MS: Amb x 4. Obese body condition. INTEG: Nails markedly overgrown and curled around. Nail on digit 5 left hind is curled around and has completely pierced webbing between digits 4-5. Nail on digit 5 left fore has punctured pad. 1 cm x 1.5 cm black, dangling skin mass lat lower left hind leg. A: Overgrown nails, secondary infected wounds. Dental disease. Pigmentary keratitis due to conformation. Resp. stertor due to brachycephalic conformation, exacerbated by obesity. Age est. 10 years. P: Rx Cephalexin 250 mg. 1 PO BID x 14 days. Rx Rimadyl 100 mg 0.5 Po q 24 hours x 7 days. Good prognosis. Will need dental cleaning when placed.
