DVM Intake Exam; Estimated age: 12 yr; Microchip noted on Intake? no; History : stray, may have fallen down some stairs on the sidewalk, no one saw him fall but he was on the sidewalk and person went inside to get some food for him and when they came out he was at the bottom of the stairs and they saw blood on his nose; Subjective: Observed Behavior – quiet, allows all handling ; Objective ; T =; P = WNL; R = WNL; BCS 2/5; EENT: Brown crust in medial canthus, OU mature cataract OS and blind in that eye, mild cataract OD, menace present, AS clean, brown debris AD, some serous discharge and ulceration with speck of blood on ventral nostrils L>R; Oral Exam: severe dental dz, all teeth covered in tartar and some are loose or already missing; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: MI sst; MSI: Ambulatory x 4, skin free of parasites, no masses noted, long toenails, dry debris stuck in fur on feet, white dry debris on front leg hair; mild sensitivity to palpation of neck, normal flexion; CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: Assessment; Geriatric; Severe dental dz; cataracts ; Possible trauma today- fell down stairs; Prognosis: Fair to good; SURGERY: Permanent waiver due to age

Look: 1. Dog’s eyes are averted, ears are back, tail is down, relaxed body posture. Dog allows head to be held loosely in Assessor’s cupped hands.
Sensitivity: 1. Dog stands still and accepts the touch, eyes are averted, and tail is in neutral position with a relaxed body posture. Dog’s mouth is likely closed for at least a portion of the assessment item.
Tag: 1. Dog stands with tail low and wagging, and comes toward the Assessor in a friendly manner when the Assessor ceases moving.
