Medical Summary DVM Intake Exam Estimated age: 2-5 Microchip noted on Intake? n History : stray Subjective: severe strangulating matts on both rear legs. matt on front legs are not medical, but are also bad. sedated with medetomidine 0.15 iv and gave .5 simbadol sq, removed matts. both legs have exposed tendons and open wounds. distal toes are pink and swollen. dubious if the feet can be saved. cleaned with chlorhex, applied TAB. Surgical debridement will be needed. Observed Behavior – wagging tail, inquisitive Evidence of Cruelty seen – yes Evidence of Trauma seen – no Objective P = 80 R = wnl BCS 1/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: mod tartar PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: intact male MSI: partially ambulatory–can stand, skin free of parasites, no masses noted,severe matting CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: severe strangulating lesion may lose one or both rear feet Plan: transfer to ASPCA or NH partner gave .4 convenia gave carprofen 4.4mf/kg once reversed with atipimizole 0.1IM Prognosis: poor for keeping feet, fair for survival Permanent waiver due to infirmity
