Medical Summary Sex: Female, intact, possibly in heat Scan: Negative Estimated age: Reported to be 1 year old; this is consistent with dentition Fleas: No flea dirt noted. Hx: Reported to be in a dog fight 2 days ago S: Passive but alert. Allows all handling. Watches everything but initially did not move on her own. After exam, was placed on the floor on a blanket, and then after a few minutes pt was willing to stand up, walk slowly around the room and then stand under the table next to examiner. O: QAR-H, BCS 4/9, MMs pink and moist, CRT <2 sec, rectal temp 103.8 EENT: Both ears are swollen at the base and there is a moderate amount of bloody dried discharge visible in the canals and on the pinnae. Small lacerations are present on pt’s dorsal skull, with dried bloody discharge. No discharge OU, nose. Mild tartar and gingivitis. H/L: NSR, NMA. Eupnic, quiet lung sounds. Abd: Soft, no pain on palpation, no masses palpated M/S/I: Amb x4 but walks stiffly on all four legs. Fur on entire body is stiff and caked with a large amount of blood and dirt. Both front legs are swollen and edematous, and appear to be abscessed along the entire antebrachium. Multiple lacerations are present on the dorsal aspect of the proximal front legs, and when the legs are squeezed gently, purulent or serous discharge is visible in the lacerations. The lacerations are dry and some granulation tissue has already begun to develop. There is also one 2-3 cm long laceration on the left lateral abdomen and one 2-3 cm long laceration on the right lateral abdomen. UG: Female, vulva enlarged and soft, no discharge noted A: 1. Many lacerations from dog fight – these wounds are likely 1-2 weeks old 2. Fever 3. Entire body is caked with blood and dirt Short-term prognosis: Fair Current surgery status: Temporary waiver for multiple injuries P: 1. Buprenorphine (0.5 mg/ml) 1 ml IM BID x2 days – first dose given during exam 2. Rimadyl 75 mg PO SID x7 days 3. Clavamox 312.5 mg (250 mg and 62.5 mg) PO BID x14 days 4. Baytril 100 mg/ml – 1.9 ml SID x10-14 days 5. Bathe in 1-2 days after pt is more stable and no longer hyperthermic, then will likely need major wound flushing and possibly drain placement 6. Hold vaccines until hyperthermia resolved 7. Monitor pt’s temperature 1088
