DVM Intake Exam; Estimated age: approx 10 yo; Microchip noted on Intake? neg; History : stray; Subjective: BAR. Does not like mouth area being touched, tried to bite. Well behaved for remainder of PE. Struggled during med handling. Observed Behavior – Objective ; T =; P = wnl; R = wnl; BCS 3.5/9; EENT: OU nuclear sclerosis and iris atrophy, OD cataract, ears clean, no nasal or ocular discharge noted. Honking cough when head/neck was restrained, and trachea palpated. Oral Exam: severe dental disease, missing most teeth; PLN: No enlargements noted; H/L: Grade IV/VI left systolic HM, NSR, Lungs clear, referred upper airway noise, normal RR/RE; ABD: Non painful, no masses palpated; U/G: MI, 2 testicles; MSI: Ambulatory x 4, skin free of parasites, no masses noted, dirty hair coat with some matting; CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: normal externally; Assessment: geriatric; HM; cataract; severe dental disease; nuclear sclerosis; iris atrophy; hair coat matting; honking cough — r/o collapsing trachea vs related to HM/cardiac disease; CBC: nsf; Chem: low glucose 62 (70-143) — r/o malnutrition; high glob 4.7 (2.5-4.5) — r/o chronic inflammation from dental disease; low choles 91 (110-320) — r/o malnutrition; Prognosis: fair; Plan: ctm until transfer/adoption; cbc/chem; clavamox 62.5mg PO BID x 10 days for the dental disease; recommend cardiology consult post placement
