I came in the shelter as a OWNER SUR on 08/10/2016 from NY 10034, owner surrender reason stated was NO TIME.

08/10/16 09:53 BASIC INFO Leydi A1084888 is an approximately 8 year old Poodle Mix. She is being surrendered to the shelter because her owner is moving out of the country. Leydi was a gift to the owner from friends 7 years ago. Leydi was last seen at a veterinarian 2 months ago. SOCIALIZATION Leydi is shy around strangers and takes time to get used to strangers. She does not play much nas she is a senior dog. Leydi lived with another small dog for about 2 weeks and she got along well with the dog. Leydi has never bitten anyone. BEHAVIOR Leydi allows you to take away her food bowl, water bowl, or toy away. She allows you to bathe her. Leydi barks if she smells or hears another dog in the hallway. Leydi struggles when you try to bathe her. FOR A NEW FAMILY TO KNOW Leydi is best described as friendly, affectionate and mellow. She has a medium activity level. When home, she follows you around. Leydi has been kept mostly indoors. She is fed Beneful dry dog food. Leidy is house trained and also uses wee wee pads. For exercise she is walked on the leash. BEHAVIOR DURING INTAKE Leydi allowed handling, collaring and photos.

Leidy came into our center as an owner surrender citing moving issues. Owner reports Leidy lived with another small dog, got along well. It is reported she is shy around strangers, takes time to warm up. Owner states Leidy is not bothered if anyone touches her Food, Toys, Treats. Owner reports Leidy is housetrained and also uses wee wee pads. Leidy walked well on leash for her relief walk, no pulling. She tried to bite on Look item, therefore remaining tactile portions of assessment were skipped. Leidy showed no interest in Toy. She must be placed with a New Hope partner to follow up on this behavior as well provide force-free based training to help her acclimate to new environments. Look 5. Dog tries to bite. Toy 1. No interest.

Sex: Female Scan: Negative Estimated age: 8 years as reported by previous owner Fleas: No flea dirt. **Per previous owner, pt is very nervous and may snap at strangers. This was not observed during the exam.** S: Nervous, slightly shaking, leaned against handler’s shoulder and was quiet during exam, allowed all exam and vx. O: BAR-H, BCS 6/9, MMs pink and moist EENT: No discharge OU, AU, nose. Moderate tartar and gingivitis, halitosis. H/L: NSR, NMA. Eupnic, quiet lung sounds. Abd: Soft, no pain on palpation, no masses palpated M/S/I: Amb x4. Fur coat thin on dorsum. hyperpigmentation over lumbar region and neck. UG: Female, small firm lumpy masses (1 cm dia) associated with right 4th and 5th mammary glands, and left 5th mammary gland. A: 1. Dental disease 2. Mammary masses 3. Hyperpigmentation on back – R/O endocrine disease vs. other cause Short-term prognosis: Fair P: 1. Administered Activyl 4-14 and pyrantel 1.4 ml PO during exam 2. Recommend dental cleaning and rads +/- extractions as needed 3. Recommend FNA of masses with excisional biopsy 4. Recommend bloodwork and evaluation of alopecia for endocrine causes 1088
