Medical Summary DVM Intake Exam Estimated age: ~ 5 yr Microchip noted on Intake? scan negative History : found stray in park Subjective: BARH Observed Behavior – timid behav, did not want to be picked up but allowed PE with minimal restraint Evidence of Objective P = 144 R = 24 BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: nsf PLN: No enlargements noted H/L: NSR, NMA, CRT ~ 3 sec; Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: intact male MSI: Ambulatory x 4; matted hair over ears, severe flea infestation with generalized thinning hair plus large areas of alopecia and thickened skin over head and caudal half of body (especially tail base and rear legs); both sides of the head have large lesions/excoriations +/- secondary bact. infection CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment – alopecia, dermatitis, fleas – r/o flea allergic dermatitis vs other Plan – Dex 0.25 ml SQ once – Simplicef 50mg PO SID x 7 days + reassess – medicated bath/groom – recheck for neuter Prognosis: good with response to tx SURGERY: Temporary waiver due to health
