Medical Summary DVM Intake Exam Estimated age: 8+ Microchip noted on Intake? neg History : O surrender – moving to apt that does not allow dogs. No other hx. Subjective: QAR-H. Appears to be somewhat visual but unable to see pupils/ PLR. Markedly long curled nails. Observed Behavior – Easily handleable. Evidence of Cruelty seen – n but long nails Evidence of Trauma seen – n Objective T = P = 100 R = eup BCS= 5/9 EENT: Eyes – cornea cloudy with keratitis changes and staining, unable to see pupils/ PLR, mild MPOD, minimal scleral hypermia. Ears clean, no nasal discharge noted Oral Exam: Marked tartar caked over premolars, gingivitis. PLN: No enlargements noted H/L: NSR, NMA, MMpk, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Spay scar seen, no tattoo. No MGT’s MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat. All nails long but approx 6 curled completely around, none embedded. Gait not affected. CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Assessment: Keratitis OU with corneal melanosis – suspect KCS Long curled nails Plan: Nails trimmed. TAB OU BID x 7 days. Re-check eyes – FS and STT. Prognosis: Fair
