I am a spayed female white and tan fox terr smooth and chihuahua sh mix. The shelter thinks I am about 13 YEARS old.
I came in the shelter as a OWNER SUR on 04/20/2017 from NY 11225, owner surrender reason stated was PERS PROB.

04/21/2017 Exam Type RE-EXAM – Medical Rating is 5 NC – SEVERE CONDITIONS NOT CONTAGIOUS, Behavior Rating is NONE, Weight 16.0 LBS.
4/21/17 Intake yesterday. Hx of seizures per owner – never had diagnostics or tx. Pale MM on initial exam. CBC/ Chem done. Hct=12%. Mild elevation LES. S) BARH. Eating well. Does not appear weak/ tired. No seizures observed since intake. O) MM pale pk, slightly icteric, crt 2. EENT: Eyes – mod nuclear sclerosis, PLR +, cornea/ sclera/ conj clr OU. Ears – cln but pinnae icteric. NND. Oral – mod tartar. MSI: BCS= 4/9, amb x 4. Soft 2 cm sq mass L flank – no painful. CV: HR= 150, NSR, slight 2/6 murmur. L: clr bilat and euo. Abd: SNP, NMP. UG: FS, no MGT’s. PLN: WNL. A) 1) Severe anemia – appears non-regenerative (Hct=12%) – r/o IMHA (less likely because non-regenerative) vs neoplasia vs bone marrow dz or other chronic dz 2) Jaundice – r/o due to anemia likely vs liver dz. 3) Mild heart murmur – secondary to anemia. 4) Seizure disorder. P) Xray WB – Chest/ Abd – no signs of masses or cancer. Stomach full. Radiodense 2 cm oblong object in R abd – r/o in gall bladder area vs stomach. Spleen not enlarged. Kidneys not visulaized. Much gas in large intestines/ colon. Repeat PCV/ TS – PCV= 15%, TS=6.6, serum not very icteric. Blood smear (unstained) – RBC dod not appear abnormal. Autoagglutination slide test – negative. (p less likely to have IMHA). Pet tinic 1.7 cc po bid. Vit B 1 complex 0.25 cc SQ, repeat tomorrow. Need rescue ASAP -dog seems vibrant/ eating well but PCV dangerously low. Recom AUS, bone marrow aspirate, Coombs test. Prognosis: guarded

04/20/17 14:06 Basic info: Laila is a 13 year old Fox Terrier/Chihuahua mix who was surrender by her owner due to personal problems. He owner adopted her from a shelter in Pennsylvania as a puppy. Laila has a history of seizures. He owner has never had her seen by the vet for this issue. Socialization: Owner stated that Laila is friendly around stranger but can be shy at times. Laila has spent time around children ages 3-17 and owner stated that she is playful and relaxed when around them. Owner also stated that when playing with adults and children Laila is gentle. Laila has not spent time with other dogs or cats. Her owner stated that when Laila sees other animals outside her will bark and run toward them. Behavior: Owner states that when left alone Laila will cry and bark at times. Owner stated that during fireworks, storms and loud noises Laila isn’t bothered. If someone touches her food or treats owner stated that Laila is friendly. When it comes to her toys Laila will growl. Owner also stated that Laila isn’t bothered if she is restrained pushed off the couch or disturbed while sleeping. Laila isn’t bothered when bathed or brushed. When someone unfamiliar approaches her family member or yard Laila isn’t bothered. For a new family to know: Owner described Laila as a friendly affectionate and playful dog with a medium activity level. Laila’s favorite activities are playing fetch with balls and tug with her rope. Owner stated Laila loves tennis balls. Laila has been kept mostly indoors and eats dry food. Laila is very much housed trained and never has accidents. When left alone in the house she is very well behaved. Lila has never been crated. She knows the sit come stay and down commands. For exercise Laila is used to brisk walks on the leash and she will use the bathroom in the grass. When off th4e leash owner stated that Laila will run away. Admissions behavior: Laila was friendly but seemed a little nervous. I was able to get her to warm up with some treats. She allowed the counselor to handle collar and take her picture.
