Medical Summary Estimated age: 14 yrs. Microchip noted on Intake? No. Found tied up to a pole at the park, no medical concerns known. Calm, friendly. Allows all handling. QARH. mm=pink, moist, CRT<2 s. HR=96. RR=20. BCS=2/9.ORAL: Multiple missing teeth. Mod to severe calculus, gingivitis. Loose L Max C and Lower incisor. EENT: Eyes clear, ears clean, no nasal discharge noted. PLN: No enlargements noted. H/L: Lungs clear, no murmurs/arrhythmias. ABD: Non painful, no masses/organomegaly palpated. U/G: Intact male, both testicles palpable. MS: Ambulatory x 4, no lameness. INTEG: Matted haircoat, debris in haircoat. CNS: Mentation appropriate – no signs of neurologic abnormalities. A: Approx 14 yr old ShihTsu with severe dental/peridontal disease, matted coat, underweight. P: Scheduled for mat shave, CBC/CHEM panel. Good prognosis with appropriate management. Will need dental cleaning and likely multiple extractions when placed. Weight 11.5
