History : Subjective: BAR Observed Behavior – friendly and docile during exam and vaccines but vocalized/growled during blood draw for HWT Evidence of Cruelty seen – large untreated ulcerated mass Evidence of Trauma seen – none Objective BCS 4/9 EENT: OU-mature cataracts; AU-mild waxy debris Oral Exam: mm lgt pk, sl tacky; CRT <2 sec; moderate tartar/staining PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: male intact, 2 scrotal testicles MSI: large, multilobulated, severely ulcerated mass above R eye; does not appear to be associated with eye or ear LH-mild decreased weight bearing when standing; chronic thickening of stifle with dec extension and intermittent click on flexion decreased hip extension bilaterally with discomfort dirty hair coat; mild flea debris CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment Large ulcerated mass-r/o sarcoma vs other L stifle thickening-suspect chronic CCL injury +/- meniscal injury with secondary arthritis Cataracts Slightly underweight Plan Rec’d CBC/chem, CXR to evaluate systemic health Rec’d excision of ulcerated mass; due to size and location, full excision may not be possible but the bulk of the mass should be removed for his comfort Convenia 80 mg/ml: 1.6 ml SQ Tramadol 25 mg PO BID until placement Prognosis: open; depends on definitive diagnosis of mass SURGERY: perm waiver due to age
