I am a male liver and white shih tzu mix. The shelter thinks I am about 7 YEARS old.
I came in the shelter as a OWNER SUR on 04/04/2016 from NY 10468, owner surrender reason stated was PET HEALTH.

04/05/2016 Exam Type RE-EXAM – Medical Rating is 4 NC – SEVERE CONDITIONS NOT CONTAGIOUS, Behavior Rating is NONE, Weight 6.2 LBS.
Hx: Client reports that pt has eye problems which require surgery. Did not say what. S: In cage, either walking in circles or lying in one place. Alert and responds to touch, but does not look around – appears blind. Eats when food is held to face but not otherwise. Had no interest in water when I offered it. O: BAR-H, BCS 3/9, MMs pink and moist, CRT <2 sec EENT: OU neovascularization, corneal opacity, scleral vessel injection, no fluorescein stain uptake. OD immature cataract, OS mature cataract. Appears blind. No discharge AU, nose. Severe tartar. PLNs: Mildly enlarged mandibular LNs. H/L: NSR, NMA. Eupnic, quiet lung sounds. Abd: Tucked up, tense. M/S/I: Decreased muscle mass on hind legs, pt drags hind feet and falls sometimes (but steps appropriately at other times). Patellar reflexes, flexor withdrawals and pain sensation in hind feet present. Circular wound on left shoulder (2 cm dia) – skin is crusty and falling off, healthy granulation tissue underneath, mild serous discharge. Fur all over body is severely matted. Urine staining on fur on ventral abdomen. UG: Male intact, testicles soft and symmetrical. A: 1. Underweight 2. Blind, cataracts, corneal scarring – possible early glaucoma 3. Hind leg neuropathy and muscle weakness 4. Healing wound on L shoulder – R/O pressure sore vs. other traumatic cause vs. infectious (ringworm) 5. Matted, urine staining 6. Dental disease 7. Only eats when food is held to face Short-term prognosis: Poor P: 1. If placed, needs a home with good nursing care – pt will need to be hand-fed, closely monitored to ensure he is eating and drinking enough, brought to pee pads etc. 2. Rec intra-ocular pressure measurement to rule out glaucoma 3. Artificial tears or Puralube OU BID x10 days 4. 04/04/16 17:18 KoKo Julian is an Approx 7 year old Shih tzu. He is being surrendered because of his health condition. He was given to the owner by a neighbor 7 years ago. His last vet visit was last week and the owner was told that he need eye surgery. KoKo Julian is blind. Socialization Koko Julian is very friendly with strangers. He has been around children and loves to play with them. He does not like to play with bigger dogs but enjoys the company of smaller dogs. He does not mind being around cats. He has not bitten anyone or any other animal. Behavior KoKo Julian has not been walked since he lost his eyesight. He uses wee wee pads in the home. He likes baths and brushing. He does not get his nails trimmed. He does not mind being carried or held. For a new family to know KoKo Julian is described as very calm, relaxed, friendly and lovable. He loves to be held and loves to be cuddled. He has a hard time walking on his own and bumps into objects in the home. He walks in circles and has to be supervised. He was fed Ceasars packets wet food. Behavior during intake KoKo Julian can not stand on his own and does not know where the edge of the table is. He was wrapped in a towel and held. He tolerated all handling. He was scanned(-) collared and photographed. He was transferred into a kennel. He walked in circles and wagged his tail.
