Medical Summary Estimated age: Approx 3-5 based on dentition Microchip noted on Intake? No History : Presents as owner surrender. Face shy- tries to bite when ears / face is examined. Allows handling with towel around neck. P = 120 R = 30 BCS 5/9 EENT: Eyes clear, Erythematous canals with yellow / blood discharge AU, no nasal discharge noted Oral Exam: Mild tartar, MM pink and moist, PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: No spay scar palpated / seen, developed mammary tissue MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat, severe interdigital swelling and erythema, nails too long CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Severe Otitis Externa Severe Pododermatitis Plan: Recommend sedation tomorrow with deep ear cleaning and application of claro (P will not allow cleaning awake). Exploration of swollen interdigital spaces Benadryl 25 mg – 2 tablets PO BID x 14 days Cephalexin 250 mg- 1.5 tablets PO BID x 14 days Prognosis: Excellent with proper medications SURGERY: Okay for surgery
