DVM Intake Exam:Estimated age: 8 years Microchip noted on Intake? scan negative
History : stray. Subjective: Observed Behavior – friendly. Objective: BCS 4-5/9 EENT: Eyes clear, AU-crusting at inside of pinnae, no nasal or ocular discharge noted Oral Exam: mild dental calc PLN: Enlarged, firm mandibular lymph nodes, pre-scapular lymph nodes, and popliteal lymph nodes H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: MI, 2 descended testicles MSI: Ambulatory x 4, skin free of parasites, approx. 1 cm pigmented cutaneous mass at left thorax CNS: Mentation appropriate – no signs of neurologic abnormalities. Assessment: enlarged lymph nodes- R/O lymphoma vs reactive vs other. Prognosis: guarded Plan: CBC/chemistry aspirate of enlarged lymph nodes: enlarged and irregular lymphocytes seen- R/O lymphoma vs other recommend further diagnostics and treatment as indicated SURGERY: Permanent waiver due to enlarged lymph nodes CBC: platelets decreased (48 K/micro/L)- R/O platelet clumping vs immune-mediated thrombocytopenia vs bone marrow disease vs other Chemistry: mildly increased ALT (134)- R/O reactive hepatopathy vs neoplasia vs other
